[Pro] PHP feedback form not working

I was using the PHP feedback form on a couple sites and everything seemed to be working ok but now my test only give me the error page when I’ve completed all boxes correctly. Any thoughts as to what might be happening? there’s a little test here at www.mahondev.com/contact.html

On this test page, your e-mail input element is actually named ‘name’ — copy/paste error, most likely. Maybe that’s related.

Help!. I’ve got the same problem. I created a form and it worked, now my second one keeps returning all sorts of code, and I’ve tried everything, except apply the php action to the table where the form site. But I can’t find out how to do that. I don’t even know if that will work. The form can be found at: http://people.aapt.net.au/~net/fincal/orders.html Any help is appreciated. Thanks.

It looks like your server is not PHP enabled.

Check with your hosting provider.
