[Pro] html form emails

Is it possible to create a ‘web survey form’ in FP for incorporation into a bulk emailing program such as Max Bulk Mailer?

If so, where can I get details on how to do?

I’m pretty sure that an e-mail client will not parse a form, or submit one somewhere. If you have an example of one that works in a broad range of e-mail clients, that would be instructive. What you can do very easily and in a foolproof manner is to host a form on a Web server, then e-mail a link to that form to your participants.
